What do I need to bring?
- Please bring a referral letter from your GP or referring doctor.
- It is important to bring any MRI scans, CT scans and X-Rays (and any reports of such) in your possession. If you have had a scan but do not have the films or report please contact the office in advance of your appointment so that we can help you to trace them.
- At the Hermitage Clinic, full direct cover is provided for all spinal procedures with the major insurers including VHI, Quinn and Vivas. Please bring details of your own health cover.
- Please bring a new consult registration form which you can download here and complete prior to your appointment
