This means opening up the bony opening and roof of the root canal as the nerve leaves the central spinal canal. It is usually done in conjunction with microdiscectomy, when there has been thickening of the facet joints and surrounding bone, to ensure that the nerve is thoroughly decompressed.

When the central spinal canal is opend up this is referred to as a laminectomy.
